Novosibirsk Institute of Traumatology and Orthopedics Tsivyan. Niito, Novosibirsk, Institute of Traumatology and Orthopedics

Activities
FSBI Novosibirsk Research Institute of Traumatology and Orthopedics named after. Ya.L. Tsivyan" of the Ministry of Health of the Russian Federation (NNIITO named after Ya.L. Tsivyan) was created as an institute of reconstructive surgery, traumatology and orthopedics (Novosibirsk VOSKHITO) in May 1946, celebrated its anniversary in 2011, having 65 years of experience behind it in the field of diagnosis and treatment of diseases of the musculoskeletal system, central and peripheral nervous system.

The institute employs about 700 employees, more than 52% of them have the highest or first qualification categories.

The institute has 9 clinical departments with 260 beds, of which 40 beds are in the children's department, diagnostic and treatment units, and 9 operating rooms. The clinics are represented by traumatology and orthopedic departments No. 1, No. 2, No. 3, No. 4, No. 5, neurosurgical departments No. 1 and No. 2, pediatric orthopedics departments No. 1, No. 2. The structure of the institute includes 9 clinical departments with 260 beds, of which 40 beds in the children's department, treatment and diagnostic units, 9 operating rooms. The clinics are represented by traumatology and orthopedic departments No. 1, No. 2, No. 3, No. 4, No. 5, neurosurgical departments No. 1 and No. 2, pediatric orthopedics departments No. 1, No. 2 and a rehabilitation center. The clinical departments of the institute provide a full range of modern medical services and treatment methods based on the latest scientific developments in the field of traumatology, orthopedics, neurosurgery - from identifying pathology, diagnostics, surgical treatment and ending with a course of rehabilitation treatment.

Innovations in the clinic
A priority place in scientific and clinical developments of the NNIITO named after. Ya.L. Tsivyan, due to their diversity, severity and the huge flow of patients, has always been interested in spinal injuries. For the treatment of spinal fractures, Ya.L Tsivyan and his students were among the first in the country to develop a number of options for anterior spinal fusion with auto- and allograft. At the end of the 70s, anterior decompression for complicated spinal fractures was first proposed instead of laminectomy, which is vicious in such cases.

Development of ideas of Ya.L. Tsivyan made it possible to radically improve treatment outcomes through the use of original ventral implants developed at the institute and abroad in combination with transpedicular fixation of the damaged spine.

Advances in modern anesthesiology today make it possible to carry out these multi-stage interventions during one anesthesia. The methods of endoscopic minimally invasive surgery used in the clinic for spinal injuries make it possible to perform corporoplasty for senile fractures without thoracotomy and thereby significantly facilitate the treatment of elderly patients.

Developed by Ya.L. Tsivyan, the operation of corrective vertebrotomy for the treatment of orthopedic consequences of ankylosing spondylitis has been performed for more than 50 years only at the Novosibirsk Research Institute of Traumatology and Orthopedics.

Since 1996, in the pediatric and adolescent vertebrology clinic of the institute, for the first time in Russia, Cotrel-Dubousset instruments, modern segmental instruments of the third generation, began to be used on a large scale, which made it possible to greatly expand the clinic’s capabilities and radically change the surgical ideology.

The problem of degenerative lesions of the spine occupies a special place in vertebrology. In 1998, a neurovertebrology department was created at the Novosibirsk Scientific Research Institute of Traumatology and Orthopedics, in which modern technologies of decompressive, stabilizing and decompressive-stabilizing operations are widely used. At the same time, a clear trend in the development of modern surgery is the development of minimally invasive surgical technologies that make it possible to minimize surgical trauma, reduce the time of inpatient postoperative treatment and postoperative rehabilitation of patients.

In the department of neurovertebrology of the institute, laparoscopic lumbar interbody fusion was performed for the first time in Russia, for which implants made of porous titanium nickelide were used. In addition to stabilizing interventions, decompressive endoscopic interventions are also performed for herniated intervertebral discs.

Neurosurgery Clinic, created by Professor K.I. Kharitonova in 1950, and is still working successfully today. The main clinical and scientific problems are neuro-oncology and vascular pathology of the central nervous system.

In collaboration with the Institute of Clinical Immunology of the Siberian Branch of the Russian Academy of Medical Sciences, the Siberian Laser Center, the Institute of Laser Physics of the Siberian Branch of the Russian Academy of Sciences, and the State Budgetary Educational Institution of Higher Professional Education "NSMU" of the Ministry of Health of Russia, new directions in the treatment of neuro-oncological patients are being developed. One of them is the use of an ND-YAG laser in the removal of brain and spinal cord tumors at key stages of microsurgical intervention. The experience of using new laser technologies in more than 1200 patients allows us to speak about their effectiveness. Neurosurgeons of the Novosibirsk Research Institute of Traumatology and Orthopedics named after. Ya.L. Tsivyan perform the most complex high-tech operations. With the help of a neuronavigator, the doctor receives the most accurate information and controls the progress of the operation. Laser surgery for tumors of the brain and spinal cord, widely used by doctors at the institute, reduces the number of relapses and the traumatic nature of the operation.

Patients with severe spinal injuries get back on their feet after surgery at the NIITO named after. Ya.L. Tsivyan in the first two days. The traumatology clinic uses minimally invasive intervention methods, combines technologies and develops its implants using new material - its properties allow the combination of spinal fixators with both biological and bioceramic grafts.

The endoprosthetics clinic operates its own production of implants; in 2011, the doctors of the department received a State Prize for the development of elbow joint endoprostheses. This development of the Novosibirsk Research Institute of Traumatology and Orthopedics received the official right to use in clinics across the country.

In the field of microvascular neurosurgery, modern microsurgical and endovascular treatment of arterial aneurysms, arteriovenous malformations and carotid-cavernous anastomoses with an original intraoperative technique for neuroprotection of the brain is successfully used, which makes it possible to effectively prevent cerebral vasospasm and secondary ischemic disorders of the brain.

One of the promising and rapidly developing areas in the Novosibirsk Research Institute of Orthopedics and Orthopedics named after. Ya.L. Tsivyana is joint replacement surgery. The endoprosthesis clinic has accumulated vast experience in using modern endoprostheses on almost all joints. Constant contacts of the staff of this clinic with spine surgeons contribute to achieving optimal treatment results for patients with combined pathology of the spine and joints.

Every year, the institute treats children with a special form of scoliosis – neurofibromatosis. In clinics, using the latest technology, children from the age of one year are equipped with special vector distractors; the rods grow with the child. In 2011, the head of the clinic for pediatric and adolescent vertebrology, Mikhail Vitalievich Mikhailovsky, was awarded the country's main medical award - the Vocation Award.

At the beginning of the third millennium, Novosibirsk Research Institute of Orthopedics named after. Ya.L. Tsivyana entered new priority areas in the diagnosis of traumatology, orthopedic and neurosurgical patients. New, including original highly informative technologies have appeared:

optical computer topography (OCT), which makes it possible to identify groups of patients in need of dynamic monitoring, conservative or surgical treatment;
X-ray densitometry;
computer biomechanics of the musculoskeletal system;
homeostasis monitoring;
clinical and genetic forecasting, which ensures obtaining accurate quantitative and qualitative indicators of health status in the shortest possible time.

Among the achievements in this area, it should be noted that to date, 226 TODP systems have been supplied to 211 institutions in 64 cities of Russia.

Specialized institutions have been created on the basis of the Novosibirsk Scientific Research Institute of Orthopedics:

All this is a real contribution to the socio-economic development of the Novosibirsk region.

In order to improve the protection and strengthening of children's health, improve the quality and efficiency of preventive medical examinations of students in educational institutions and early detection of spinal deformities, since 2010, students of educational institutions in the city of Novosibirsk have been examined using a computer-optical topographer (hereinafter referred to as COMOT). The examination is carried out by specialists from the ANO Clinic NIITO with the support of the government of the Novosibirsk region and the mayor's office of Novosibirsk. More than 70 thousand schoolchildren in the region underwent spinal screening examination using the COMOT method.

Over the past five years, the Novosibirsk Scientific Research Institute of Traumatology and Orthopedics has been a leader in providing high-tech medical care to the population of the Novosibirsk region and other regions of Russia (mainly from the Ural, Siberian and Far Eastern federal districts). The total number of operations today is more than 8 thousand per year, of which more than 50% occur among residents of the city of Novosibirsk and the Novosibirsk region.

Innovation in science
Today Novosibirsk Scientific Research Institute of Orthopedics named after. Ya.L. Tsivyana is not only the All-Russian Center for providing specialized medical care to traumatology and orthopedic patients, but also the largest scientific institution and methodological center in Russia in the field of traumatology and orthopedics.

Over the course of many decades, the institute has developed and improved a number of original, effective surgical methods for treating patients. The scientific profile of the institute is associated with the names of well-known scientists - professors Ya.L. Tsivyan and K.I. Kharitonova, who created schools of Siberian vertebrologists and neurosurgeons.

Today, the developments of employees of the Novosibirsk Research Institute of Orthopedics named after. Ya.L. Tsivyan on the problems of spinal pathology are reflected in 46 monographs, 160 candidate and doctoral dissertations, monothematic collections of scientific papers, protected by 220 copyright certificates and patents and awarded gold, silver and bronze medals; More than 75 new medical technologies, 33 standards of medical care in vertebrology, 13 federal clinical recommendations have been developed and approved.

Every year the institute plans to conduct about 40 research and development projects in the following areas:

Scientific potential of NNIITO named after. Ya.L. Tsivyan today: 13 professors, 27 doctors of science and 64 candidates of science.

The institute has created a modern scientific, experimental and experimental production base, which currently allows us to bring the developed experimental samples to the level of medical products. The institute has organized a bank of biological tissues, which is in great demand among specialists in the field of medicine not only in our country, but also abroad. In addition, the Institute is a co-executor of a number of programs to develop and improve new promising areas of research in vertebrology, including projects: the creation of high-tech production of implant-fixators from nanostructured biocompatible ceramics and ceramic composites replacing metal implants; development of artificial biopolymer coatings and sorbents to neutralize para-implant pathogenic microflora on the surface of implanted devices and systems; development of biomaterials for bone grafting, etc.

By developing its own production, the institute has largely ceased to depend on foreign manufacturers of surgical instruments and implantation structures, which also brings a tangible socio-economic effect. With the support of the government of the Novosibirsk region, a project has been implemented to create an Innovative Medical Technology Center, which contributes to the creation of new organizational forms for using the scientific achievements of the institute in practice and more actively introducing the results of its scientific activities into practical healthcare.

Innovations in education
Novosibirsk Research Institute of Traumatology and Orthopedics is a large educational center for postgraduate education, where, on the basis of a license for educational activities within the framework of the federal state standard of the third generation using simulation technologies, residents and graduate students annually receive a specialty in the profiles of “traumatology and orthopedics”, “neurosurgery”, “anesthesiology and resuscitation”. ”, and traumatologists-orthopedists and neurosurgeons of Russia also improve their qualifications within the framework of postgraduate education.

One of the most important issues that the institute solves is the issue of training personnel to provide high-tech medical care that meets international standards, which allows cooperation with leading experts in the world (Switzerland, France, Germany, Italy, USA), as well as holding international conferences on its basis , seminars and master classes.

Innovation in the organization
For more than a decade, the country's leading orthopedic scientists have been raising the question of making vertebrology a separate specialty. At the Novosibirsk Scientific Research Institute of Traumatology and Orthopedics, a theoretical justification for this specialty was given, a conceptual apparatus was formulated, a scheme for organizing a specialized assistance service was proposed, technology standards were developed and the needs of the service were formulated. By order of the Ministry of Health of the RSFSR dated April 12, 1987 No. 257, the Republican Vertebrology Center was created at the institute, which in 1999 was renamed the Center for Spinal Pathology of the Ministry of Health of Russia. In May 2009, the Interregional public organization “Association of Spine Surgeons” was created, and the director of the institute, Doctor of Medical Sciences, Professor Mikhail Anatolyevich Sadovoy, was appointed president of the organization.

On the basis of the institute there is a dissertation council for the defense of doctoral and candidate dissertations in the specialties of “traumatology-orthopedics”, “neurosurgery”, “public health and healthcare”. Since 2004, the Russian scientific and practical journal “Spine Surgery” has been published, which was included in the list of publications of the Higher Attestation Commission in 2005, recommended for publication of the main provisions of dissertation research. All this creates the best conditions for the development of Siberian scientific schools, allows for the formation of a new generation of scientists and clinicians of the highest professional level, which is closely related to the growing needs for the provision of all types of medical care in traumatology, orthopedic and neurosurgical profiles.

Novosibirsk Scientific Research Institute of Orthopedics named after. Ya.L. Tsivyana is actively involved in the development and implementation of innovations in the healthcare sector, primarily organizational ones. At the same time, the emphasis in innovative development is largely placed on the introduction of the most modern approaches to management and building an effective medical organization.

The institute came close to creating an effective management system for a medical organization back in 2002. According to scientists, the system was supposed to solve issues of not only improving the quality of medical care, but also increasing its accessibility for the population as a whole. The result of two years of preparatory work was the certification of the quality management system in 2004 and the receipt of an international certificate of compliance with the requirements of the ISO 9001 standard. Currently, the institute has moved from a quality management system to a more advanced system - to a system of sustainable development of a medical institution.

The model of the quality management system for healthcare institutions developed by the Novosibirsk Scientific Research Institute of Traumatology and Orthopedics has been implemented and successfully used in a number of medical organizations and medical industry enterprises of the Russian Federation, including the State Autonomous Institution "Children's Republican Clinical Hospital of the Ministry of Health of the Republic of Tatarstan" (Kazan), the State Autonomous Institution of the Republic of Tatarstan "Hospital emergency medical care" (Naberezhnye Chelny), Federal State Institution "Siberian District Medical Center" (Novosibirsk), Federal State Public Institution "Scientific and Clinical Center for Miners' Health" (Leninsk-Kuznetsky), Federal State Unitary Enterprise "Novosibirsk Prosthetic and Orthopedic Enterprise" Ministry of Labor and Social Protection of the Russian Federation, Avicenna Medical Center CJSC (Novosibirsk), Technology-Standard LLC (Barnaul), Laboratory Diagnostics Center LLC (Novosibirsk), Regional Center ANO High Medical Technologies", CJSC "Production Pharmaceutical Company Obnovlenye" (Novosibirsk), LLC "Medical Engineering Center for Shape Memory Alloys" (Novosibirsk). Novokuznetsk).

Currently, work continues to create a unified information space in the field of providing patients with all types of traumatological and orthopedic care, thereby resolving the issue of supervising the most severe patient population.

With the participation of the Novosibirsk Scientific Research Institute of Traumatology and Traumatology, the first Medical Technopark in Russia was opened in Novosibirsk - the Innovative Medical Technology Center. The structure of the Medical Technology Park provides all the necessary infrastructure for the development of innovative medical technologies, allowing an innovative medical company to go all the way from transforming its scientific idea into a competitive medical product or service.

The widest range of professional and social activities of employees of the Novosibirsk Research Institute of Orthopedics named after. Ya.L. Tsivyana undoubtedly produces results for combining efforts to improve traumatological and orthopedic care to the population and makes a great contribution to the socio-economic development of not only the Novosibirsk region and the Siberian Federal District, but also other regions of the Russian Federation.

I broke my hip 4 years ago, I’ve been going to this clinic for 3 years, every year they reschedule for a year, the upper part of the bone crumbled into pieces during this time, soon I’ll go there again, I don’t count on anything else, unfortunately I made a mistake in my choice clinics. For us, our injuries are life, but for all these 100+ Ph.D. With all their equipment, we are nothing. I go and immediately take a return ticket, since I know a priori that this will be another refusal for another year. Now I am collecting documents to be sent to the RNIITO named after. R.R...

Repin Andrey Vasilievich! She was admitted on May 10, 2017 with an improperly healed, or rather non-united, ankle fracture. He explained everything and warned about everything. He operated on the next day after admission. The attitude is great! Every day I came in to find out how I was feeling! An excellent specialist and a very pleasant person.

Reinforcing pins stick out from the walls of the clinic, so strong that today (April 14, 2017) my husband suffered an arm injury and tore his leather jacket. The security saw it and didn’t react at all! From a telephone conversation with Sergei Olegovich Ladkin (he did not disclose his position), it turned out that they know about the existence of the hook pin, but no measures have been taken to avoid accidents and eliminate the risk of injury to NIITO visitors. From the side of Ladkin SO, only memorized phrases came out -> I don’t know anything, there is no manual, to...

On November 21, 2016, I signed an Agreement for the provision of paid medical services, not actually for services, but for collecting money in advance of 4,000 rubles. On the same day, doctor Alexander Vladimirovich Shestakov performed “treatment” for 800 rubles with shock wave therapy for 1 minute 45 seconds (he looked at his Casio watch). Is this a treatment? Moreover, the doctor stated that he does not give any guarantees! On November 23 at 13-30 Shestakov A.V. again “treated” for about 2 minutes using an UVT device, but I will not pay 800 rubles for such...

On August 19, 2016, a hip replacement operation was performed. It started with anesthesia; they couldn’t get into a vein, which they blamed on me (i.e., the patient). During the operation, I asked to be put to sleep so as not to hear the prosthesis being driven into the bone with a hammer, but it never happened. But this is nothing compared to what I encountered in the intensive care unit! The nurse on duty 08/19/16 is a complete boor without any signs of humanity. When I asked for pain relief, she was rude, rude, and called me inadequate! How...

Guys, you're a little greedy! On Sunday, September 18, I called on a multi-line phone. The administrator (did not introduce himself) put him through to the on-duty traumatologist Borzykh (surname matches), who somehow REALLY didn’t want to work on Sunday. My relative has a spinal process fracture. On Saturday they looked at him at the city hospital and kicked him to the Scientific Research Institute. But my relative does not have a policy, he works in the Ministry of Internal Affairs, he needs to wait until Monday, when the State University clinic opens and gives a referral to the Research Institute of Traumatology and Orthopedics. But...

I would like to express my deep gratitude to the doctor Timofey Igorevich Alexandrov, as well as to everyone who took part in the elbow joint replacement operation of Natalya Alexandrovna Aksinina. The hand is now working. You are simply WIZARDS, THANK YOU so much for this. I also want to say a huge THANK YOU to the anesthesiologist and bronchoscopist who helped her survive her condition after a tracheostomy. The operation took place on April 8, 2016, and now the arm is working normally. THANK YOU everyone, you are simply wizards with a capital W.

She had surgery in July 2014. Yes, there is a doctor and honey in the waiting room. sister is rude. I sat there for about an hour until I got through to the doctor that I was from another city and my name was not on his list. I thought it was like this throughout the hospital, but I was very mistaken. All honey The staff is responsive and does their job very well. They will always listen, help, and advise. They come up and ask if they need anything. I never heard a rude word from them. Thank them very much, from the bottom of my heart! To the doctor, Belozerov V.V., a separate...
