Admission to the College for training in educational programs at the expense of budgetary allocations of the Samara region is publicly available, unless otherwise provided by Federal Law, the volume and structure of admission of persons to the College for study at the expense of budgetary allocations of the Samara region are determined in the manner established by the Founder in accordance with the legislation of the Russian Federation in the field of education. The college has the right to admit students in excess of the established budget places for training on the basis of agreements on the provision of paid educational services. At the same time, the total number of students in an educational institution should not exceed the maximum number established in the license for the right to conduct educational activities. II. Organization of admission to the College 2.1. Admission to educational programs is organized by the College's admissions committee (hereinafter referred to as the admissions committee). The work of the admissions committee and office work, as well as personal reception of applicants and their parents (legal representatives) are organized by the executive secretary of the admissions committee, who is appointed by order of the director of the College. In the branches of the College, work admission committee and office work, as well as personal reception of applicants and their parents (legal representatives) are organized by the deputy chairman and deputy executive secretary of the admission committee, who are appointed by order of the director of the College. To organize and conduct entrance tests in specialties that require applicants to have certain creative abilities, physical and (or) psychological qualities (hereinafter referred to as the entrance examinations), the chairman of the admissions committee approves the composition of the expert and appeal commissions. Upon admission to the College, the rights of citizens in the field of education established by the legislation of the Russian Federation are ensured, the work of the admissions committee is transparent and open in order to confirm the authenticity of the documents. submitted by applicants, the admissions committee has the right to contact the relevant state (municipal) bodies and organizations. III. Organization of informing applicants 3.1. The College announces the admission of citizens for training in educational programs in accordance with the license to carry out educational activities. IV. Acceptance of documents from applicants 4.1. Application for admission to the College submitted in Russian When submitting an application for admission, citizens of the Russian Federation provide the following documents: original or photocopy of documents certifying their identity and citizenship; original or photocopy of education document; 4 photographs (3x4 cm) The Admissions Committee also recommends that you provide additionally: when applying for full-time or part-time study - a photocopy of your work record book; for young men and those liable for military service - a photocopy of the registration certificate or military ID. Foreign citizens, stateless persons, including compatriots living abroad, provide: a copy of the applicant’s identity document, or an identity document a foreign citizen in the Russian Federation, in accordance with Article 10 of the Federal Law from the Federal Law “On the legal status of foreign citizens in the Russian Federation”; the original of a state-issued document on education and (or) qualifications (or its certified copy), if the education certified by the specified document is recognized in the Russian Federation at the level of the corresponding education in accordance with Article 107 of the Federal Law; a certified translation into Russian of a document of a foreign state on the level of education and (or) qualifications and its annexes (if the latter is provided for by the legislation of the state in which such a document was issued); copies of documents or other evidence confirming that a compatriot living abroad belongs to the groups provided for in Article 17 of the Federal Law of the Federal Law “On the state policy of the Russian Federation regarding compatriots abroad”; 4 photographs (3x4 cm). The last name, first name and patronymic (last if available) of the applicant, indicated in the translations of the submitted documents, must correspond to the last name, first name and patronymic (last if available) indicated in the identity document of the foreign citizen in the Russian Federation. In the application, applicants indicate the following mandatory information: - last name, first name and patronymic (the latter - if available); - Date of Birth; - details of the document proving his identity, when and by whom it was issued; - information about the previous level of education and a document on education and (or) qualifications confirming it; - specialty/profession for which he plans to enter the College, indicating the conditions of study and the form of education (within the test Page 8 of 14 in

The application also records the fact of familiarization (including through public information systems) with the Charter of the College, copies of the license to carry out educational activities, certificates of state accreditation of educational activities and appendices thereto. The fact of familiarization is certified by the personal signature of the applicant. The applicant’s signature also confirms the following: - receiving secondary vocational education for the first time; - familiarization (including through public information systems) with the date of provision of the original document on education and (or) qualifications; - consent to the processing of your personal data in the manner established by Federal Law No. 152-FZ “On Personal Data”. If an applicant submits an application that does not contain all the information provided for in this paragraph and (or) information that does not correspond to reality. The college returns the documents to the applicant. According to the decree of the Government of the Russian Federation, the applicant provides the original or a copy of a medical certificate containing information about a medical examination in accordance with the list of medical specialists, laboratory and functional tests established by Order of the Ministry of Health and Social Development of the Russian Federation 302n. A medical certificate is recognized as valid if it is received no earlier than a year before the end of the acceptance of documents and entrance examinations. In the event that an applicant fails to provide or is invalid a medical certificate, or if it does not contain, in whole or in part, information about a medical examination that meets the requirements established by Order of the Ministry of Health and Social Development of the Russian Federation 302n, the College ensures that the applicant undergoes a medical examination in full or in the missing part in the manner established by the said order. Information about the time and place of the medical examination is posted on the official website of the College. If an applicant has medical contraindications established by Order of the Ministry of Health and Social Development of the Russian Federation 302n, the College provides him Page 9 of 14

10 informing about the consequences associated with the specified contraindications during the period of study at the College and subsequent professional activities. Applicants have the right to send an application for admission, as well as the necessary documents through public postal operators (hereinafter by mail) in accordance with the Federal Law of the FE “On electronic signature”, Federal Law from the Federal Law “On Information, Information Technologies and Information Protection”, Federal Law from the Federal Law “On Communications”. When sending documents by mail, the applicant encloses with the application for admission photocopies of documents proving his identity and citizenship, education document, as well as other documents provided for by these Rules. Documents sent by mail are accepted upon their receipt by the College no later than the deadlines established by clauses 4.3 and 4.4 of these Rules. When submitting original documents in person, applicants are allowed to have their photocopies certified by the College. The basis for refusing to accept documents to an applicant may be: provision of an incomplete set of documents, in accordance with these Rules; provision of unreadable copies of documents In the event of a violation of the Procedure for Admission to the College, which, through the fault of the student, resulted in his illegal enrollment in the College, educational relations may be terminated early at the initiative of the College (Federal Law, Article 61, paragraph 2) It is not allowed to charge applicants when applying documents specified in paragraph of these Rules A personal file is opened for each applicant in which all submitted documents are stored. When submitting documents in person, the applicant is given a receipt for documents acceptance. Upon written application, applicants have the right to pick up the original education document and other documents submitted by the applicant. Documents must be returned by the College within the next business day after submitting the application. Page 10 of 14

11 V. Entrance tests 5.1. In accordance with the list of entrance tests for admission to educational programs of secondary vocational education in specialties that require applicants to have certain creative abilities, physical and (or) psychological qualities, approved by the Ministry of Education and Science of the Russian Federation, when applying for training in the following specialties of secondary professional education: “General Medicine”, “Midwifery”, “Nursing”, “Orthopedic Dentistry” entrance tests are held Entrance tests are held: in the specialties “General Medicine”, “Midwifery”, “Nursing” - psychological testing in writing form; in the specialty “Orthopedic Dentistry” - sculpting. Entrance tests are held from August 1 to August 15, 2015. The schedule of entrance examinations is communicated to the applicant at the admissions committee on the day of application. To be admitted to the entrance examination, you must have a passport and a receipt indicating the day of passing the entrance examination, which is issued by the admissions office. Repeated passing of the entrance test to increase the score is not allowed. If, due to illness or other good reason, the applicant was unable to attend the entrance examination, then upon presentation of the appropriate certificate before the end of the entrance examination, he may be allowed to take the missed examination on another day. Entrance examinations are conducted on the basis of entrance examination programs approved by the Chairman of the Admissions Committee. The procedure for conducting and evaluation criteria for entrance tests are posted on the information board of the admissions committee and on the official website. VI. Peculiarities of conducting entrance tests for persons with disabilities 6.1. Persons with disabilities, when entering the College, take entrance tests taking into account the characteristics of psychophysical development, Page 11 of 14

12 individual capabilities and health status (hereinafter referred to as individual characteristics) of such applicants When conducting entrance tests, the following requirements are ensured: entrance tests are conducted for persons with disabilities in the same classroom together with applicants who do not have disabilities, if this does not create difficulties for applicants upon passing the entrance test; the presence of an assistant who provides the applicant with the necessary technical assistance, taking into account his individual characteristics (take a workplace, move around, read and complete the assignment, communicate with the examiner); applicants are provided with printed instructions on the procedure for conducting entrance tests; applicants, taking into account their individual characteristics, can use the technical means they need during the entrance test; material and technical conditions must ensure that applicants have unhindered access to classrooms, toilets and other premises, as well as their specified premises. VII. General rules for filing and consideration of appeals 7.1. Based on the results of the concessional test, the applicant has the right to submit a written statement to the appeal commission about a violation, in his opinion, of the established procedure for conducting the test and (or) disagreement with its results (hereinafter referred to as the appeal). I 7.2. Consideration of the appeal is not a retake of the entrance test. During the consideration of the appeal, only compliance with the established procedure for conducting the entrance test is checked. The appeal is submitted by the applicant in person the next day after the announcement of the grade for the entrance test. In this case, the applicant has the right to familiarize himself with his work performed during the entrance test in the manner established by the College. The admissions committee ensures that appeals are received throughout the working day Page 12 of 14

N Lyapina" Consideration of appeals is carried out no later than the next day after the day of familiarization with the work performed during the entrance examination. When considering appeals, representatives of the executive authorities of the Samara region, exercising public administration in the field of education, are included as independent experts in the appeal commission. The applicant has the right to be present when considering an appeal. I The applicant must have an identification document with him. One of the parents or other legal representatives has the right to be present with a minor applicant. After consideration of the appeal, a decision is made by the appeal commission. If disagreements arise, a vote is held in the appeal commission, and the decision is approved by a majority vote. The decision of the appeal commission, documented in the protocol, is brought to the attention of the applicant (against signature). VIII. Enrollment in College 9.1. The applicant submits the original document on education before August 15, 2015. Upon expiration of the deadline for submitting original documents on education, the director of the College issues an order for the enrollment of persons. recommended by the admissions committee for admission and who have submitted the originals of the relevant documents. Persons who have submitted an application for admission, passed a competitive selection and provided the original document on education within the deadlines established by the College are eligible for enrollment. If the number of applicants exceeds the number of places financially supported by budgetary allocations of the Samara region, the College provides admission to educational programs of secondary vocational education in specialties based on the results of the applicants’ mastery of the educational program of basic general or secondary general education specified in submitted by incoming state documents on education. This educational institution can be considered one of the oldest secondary schools in our country, because it began its activities at the end of the 19th century. Historical reference

The medical college that now exists in Samara was founded back in 1867. In January, it was decided to open a small educational institution. It appeared in May of the same year. They called it the paramedic-midwifery school. The educational institution began operating at the zemstvo hospital. In the first year of work, 15 people entered here.

For approximately 39 years, the school trained paramedics and midwives. In 1906, her status and profile changed. The educational institution became a medical college. The new task that he faced was to train 4 types of specialists:

  • infant and maternity care nurses;
  • assistant sanitary doctors;
  • general nursing nurses;
  • midwives.

Transformations of the educational institution and naming of the name Nina Lyapina

Until the mid-50s, 2 transformations occurred with the educational organization in question. It again became a paramedic-midwifery school, and a little later it was named a medical school. In 1967, the educational institution celebrated its anniversary - 100 years since its foundation.

In connection with such a significant event, the school was named after Nina Lyapina. This woman was a graduate of this educational institution. In 1940, she graduated and went to the front to help the wounded. Nina Lyapina saved soldiers and was ready to give her life for her Motherland. Unfortunately, this brave, brave woman did not return from the front. In one of the battles she was mortally wounded.

Educational institution today

Currently, the medical school that once existed in Samara continues to operate, but it has a different name. The educational institution is a college. This status was assigned to the college in 1993. Today the college is one of the leading secondary specialized educational organizations. In secondary school there is also a specialty “Optics”. It is not found in every medical secondary educational institution in our country. Samara Medical College named after. N. Lyapina is one of the oldest in the country and the first special institution in Samara.

On January 11, 1866, at a meeting of the Samara Provincial Assembly, a decision was made to approve a paramedic school and a school of midwives at the provincial hospital.
